Job Description
Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. This role involves performing clinical assessments, administering medications and treatments, and supporting patients' physical and emotional well-being. The ideal candidate will possess a strong foundation in nursing practices, excellent communication skills, and a commitment to delivering patient-centered care in accordance with regulatory standards. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories, ensuring accurate documentation within electronic health records (EHR/EHR systems). Administer medications, injections—including IV infusions—and treatments following physician orders while adhering to safety protocols such as sterile technique and aseptic procedures. Monitor patients for signs of acute pain, infection, or deterioration; implement appropriate interventions and escalate concerns as necessary. Assist with specialized care procedures such as catheterization, tube feeding, wound suturing, and ventilator management. Provide support in emergency situations through first aid, airway management, and CPR; participate in triage for urgent care needs. Collaborate with interdisciplinary teams to develop and implement individualized care plans for diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities or dementia. Maintain compliance with HIPAA regulations and confidentiality policies while managing medical documentation accurately. Support discharge planning and coordinate with case management teams to ensure seamless transitions of care. Utilize EMR/EHR systems such as Epic or Cerner for documentation; ensure timely entry of clinical notes and medication administration records. Educate patients and families on health management strategies including medication usage, nutrition, and post-discharge care instructions.
