Local Rate:
$40/hour on W-2 Position Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a 13-week contract opportunity in New Lisbon, Wisconsin , with the possibility of extension. The LPN will provide quality nursing care to residents, administer medications, monitor changes in condition, communicate with physicians and families, and collaborate with CNAs and other members of the healthcare team. Key Responsibilities Administer medications to residents while following the 5 Rights of Medication Administration . Monitor residents and promptly report any changes in condition to the charge nurse. Notify physicians and family members regarding changes in a resident's condition as appropriate. Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator. Assist CNAs with answering call lights and other resident-care responsibilities as needed. Maintain accurate and timely documentation. Communicate effectively with residents, families, physicians, nurses, CNAs, and other healthcare professionals. Follow facility policies, procedures, and applicable regulations. Perform other duties as assigned. Required Qualifications Active Wisconsin LPN License — Required Current CPR/BLS Certification — Required 1+ year of related LPN experience — Preferred Strong attention to detail and exceptional accuracy Excellent communication and interpersonal skills Ability to work effectively as part of a healthcare team Driver's License (DL) copy required for submission Two professional references required
