POSITION SUMMARY
: Under the supervision of the Associate Director of Quality Management, the Population Health Medical Assistant provides agency wide support for managers and teams to meet and exceed the organization's quality standards and expectations. The Population Health Medical Assistant is responsible for patient education, support for various quality incentive measures and assistance with data quality assurance tasks. In addition, they are expected to be able to perform all the functions of a Care Team Medical Assistant as identified on the Care Team Medical Assistant job description when needed at specific clinical shifts. This position would require commuting between our clinics, located in Sebastopol, Occidental, and Guerneville.
Duties and Responsibilities:
Contact patients for preventative care and services via mailers, phone calls, text, email campaigns, community events, groups, or other methods needed to establish contact and build effective connection with patient population. Assist with patient notification letters and patient provider reassignments as needed. Assist in identifying patient barriers and identification of supportive services and resources available to patients to meet specific needs. Assist patients to access and navigate medical, behavioral health, dental, and other specialty care, as needed. Assist patients to connect with supportive services and resources both within WCHC and throughout the community that are critical to their overall health and well-being. Assist with select Quality Improvement/Quality Assurance or other projects as directed to support teams in achieving goals and objectives. Assist the Associate Director of Quality Management with data collection and validation. Assist the Associate Director of Quality Management with projects related to data integrity. Prepare exam rooms before and after each patient in accordance with WCHC policy and OSHA guidelines, including restocking of exam rooms and operatories. Assist with clinical data measure and entry into the electronic health record including the patient's height, weight, and vital signs including pulse rate, temperature, blood pressure, respiratory rate, BMI, etc. Actively manage the flow and experience of the patients during the office visit. Perform in-house lab tests (HCT, rapid strep, urine dips/spins, Hgb A1C, serum glucose, urine pregnancy, etc.) See that correct billing information is sent out with specimens for off-site lab tests and maintains log of all specimens. Perform vision and hearing tests, EKG's, ABT's, ear irrigation, immunizations, dressing applications and other activities as requested. Perform venipuncture (drawing blood) using proper technique and sterile procedure, ensuring proper specimen collection and handling for laboratory analysis. Work collaboratively with the Care Team to complete needed patient care items. Assist medical providers with exams and procedures as requested (i.e., colposcopies, biopsies, vasectomies, pelvic exams, circumcisions, casting, etc.). Assist in discharging the patient at the completion of the office visit including going over the printed office visit summary; aid in the completion of medication, laboratory, and radiology orders; provide patient education information ordered by the provider; answer any final logistical questions the patient may have; and assist in follow-up appointment.
Education:
Completed an accredited Medical Assisting program Certification preferred, but staff are encouraged to be certified within one year of hire date Current CPR certificate.
Skills and Experience:
Bilingual English and Spanish desired. Experience in an outpatient setting or working with a health clinic. Experience working with an electronic health record (EHR) system highly desirable. Computer literacy and familiarity with the Microsoft Office Suite, particularly Excel. Basic office and equipment safety knowledge and practice.
Physical Requirements:
Ability to bend and move arms with sufficient range of motion to complete patient related requirements, hearing required for patient assessments, may need to assist patients onto exam table, able to lift 30 lbs., bend, kneel and twist as needed. Prolonged standing and occasional sitting required. Must be able to perform the essential duties with or without accommodations.
