Job Description

PATIENT CARE / REFERRAL COORDINATOR
We are expanding our staffing in this important position that provides much needed extra support to our most at-risk patients. You will also work with our general patient population to support obtaining routine primary care. This position will work in both our Price and Huntington locations and will be cross-trained to work in our urgent care as needed. We are hiring two part-time or one full-time candidate depending on applicant pool. Please specify if you prefer part-time or full-time. Willing to train the right candidate.
Qualifications:
1. 2+ years of experience as a Medical Assistant 2. Proficient in anatomy, physiology, medical terminology, and pharmacology. 3. Experience with CPT codes and ICD-10 codes. 4. Experience with health insurance plans, prior authorization requirements, and referral process preferred. 5. Excellent customer service skills. 6. Excellent telephone etiquette. 7. Excellent interpersonal skills, including friendliness, empathy, patience, kindness, politeness, and helpfulness. 8. Ability to build rapport with community partners in the medical neighborhood. 9. Ability to work independently with a strong sense of focus. Self-starter. 10. Task-oriented, strong organizational skills, ability to multitask. 11. Phlebotomy and limited license x-ray 12. Strong attention to detail. 13. Skilled in basic computer operations and EMR.
Responsibilities:
1. Coordinate medical specialty referrals and procedures for patients, as assigned. 2. Process necessary procedure and imaging prior authorizations and insurance referrals as assigned. 3. Process necessary medication prior authorizations as assigned. 4. Track patient visits to the ER and follow-up using set standards. 5. Work with patients in our chronic care management program to help them obtain their health goals. 6. Assist physicians with Medicare Annual Wellness Visits by administering multiple screening tests, taking vital signs, administering injections, and preparing patients for examination. 7. Orders clinical supplies. Monitors and ensures the accuracy of clinical inventory levels including VFC immunization inventory. Serves as coordinator for the VFC program. 8. Assists in the care and maintenance of equipment including running daily controls on in-office lab equipment. 9. Processes incoming faxes according to set document standards. 10. Occasional evening and weekend shifts in our urgent care. 11. Other duties as assigned.
