Compensation & Benefits:

Responsibilities:
Comprehensive patient assessments and ongoing care management Manage acute and chronic conditions Coordinate care with interdisciplinary teams Maintain timely documentation and quality standards
Qualifications:
BC|BE in Internal Medicine, Family Medicine, or Geriatrics New York license (or ability to obtain)
