Job Description

Education:
High school diploma or GED equivalency; basic medical terminology required; one (1) year of relevant experience within the last seven (7) years working in a medium-to-large medical facility may be accepted as an equivalency for the education requirement.
Certifications:
Valid BLS certification is mandatory; RHIT or RHIA certification is required in addition to Basic Life Support.
Experience:
Minimum one (1) year of relevant experience within the last five (5) years working in a medium-to-large medical facility; experience with EHR systems required.
Medical Records Duties:
Respond to inquiries from patients, authorized representatives, law firms, insurance companies, government agencies, researchers, and other third parties; verify release-of-information authorization requirements in accordance with AR 40-66, HIPAA, and applicable guidance; release health information; abstract medical record information; obtain supplemental reports; prepare factual summaries and administrative forms for provider signature.
Correspondence:
Receive and screen incoming correspondence requesting medical information; prepare correspondence; communicate effectively both orally and in writing.
Records Review:
Perform periodic medical records reviews in accordance with AR 40-66 and The Joint Commission Handbook to ensure regulatory compliance.
Records Retrieval:
Request medical records from military and civilian facilities by written or fax request; coordinate with facilities outside BAMC by telephone for urgent record retrieval; screen, log, and forward incoming medical records to the appropriate physician, clinic, or records file room.
Legal Requests:
Receive and process subpoenas, investigative requests, court summons, risk management reviews, and other official/legal requests; prepare certified copies of medical records; coordinate with the Staff Judge Advocate before releasing information in accordance with AR 40-66 and HIPAA.
Records Management:
File and retrieve records; copy and scan medical records; convert records to electronic media; prepare cover letters and billing vouchers; process records for medical boards and VA claims; assist with medical profile processing.
Discharged Patient Records:
Process discharged inpatient and ambulatory patient records; retrieve records from hospital wards; print electronic records; merge documents and electronic records in the prescribed order; follow AR 40-66 and local policies.
Record Accountability:
Print discharged-patient reports from
MHS GENESIS
; retrieve records from hospital wards; perform 100% accountability of discharged patient records; coordinate with unit clerks and head nurses to obtain missing records or reports; report records not located within two days of discharge to the Department Chief.
Quality Review:
Perform quality and quantitative reviews of discharged patient records; verify completeness and required documentation; ensure reports are included and signed by providers; coordinate with physicians and nursing staff to obtain missing reports or signatures.
Incomplete Records:
Track incomplete medical records; prepare and publish monthly provider reports from

; distribute pending-record lists to providers; clear completed records from reports and

; monitor outstanding records and report issues to the Department Chief.
Transportation:
Drive and operate a government vehicle to transport medical records between the hospital and Patient Administration Division activities.
Support and Guidance:
Provide information to paraprofessionals, physicians, and nursing staff regarding medical record procedures and policies in accordance with established guidance.
