Job Description

Key Responsibilities Medication Management & Documentation Prepare, administer, and document medications in accordance with Mosaic policies, physician orders, and state regulations. Maintain accurate and timely entries in the Medication Assistance Record (MAR). Verify, clarify, and update physician orders as needed. Order, receive, and securely store medications in compliance with community protocols. Maintain a clean, organized, and secure medication room. Resident Care & Support Assist residents with activities of daily living, including bathing, grooming, dressing, toileting, and ambulation. Take and record vital signs, monitor health changes, and communicate updates promptly to the Clinical Manager or Executive Director. Provide first aid and wound care when necessary and coordinate care with outside healthcare professionals as directed. Qualifications & Skills Caregiver certification required High school diploma or equivalent required Previous long-term care experience, preferred. Strong organizational skills and attention to detail. Ability to work independently, follow procedures, and prioritize tasks in a fast-paced environment. Excellent written and verbal communication skills in English. Must demonstrate empathy, patience, and a strong commitment to resident dignity. Flexibility to work evenings, weekends, and holidays as needed.
