We take care of the highest cost, highest risk patients of AHCCCS and Medicaid plans, so they have high needs. You will work with these members in their homes to coordinate their care, educate them on medical needs, ensure they have proper conditions for successful health and engage them around their goals toward a better life. You will work closely with their Primary Care Providers, Specialist, Case Managers, and others to ensure their success. All members are managed for 1 year with the goal to improve their medical outcomes. We see these members in their homes so that we reduce their burden of travel and increase our knowledge of the barriers existing in their home.
Responsibilities:
  • Examine patients and their medical records
  • Review diagnostic tests
  • Create treatment plans incorporating integrated medical practices
  • Prescribe medications
  • Reconcile medications
  • Maintain accurate encounter records
  • Educate patients and their family members around their medical needs
  • Oversee patient's overall engagement with healthcare providers
  • Collaborate with patients' providers around care planning and next steps
  • Administer transitional care procedures and education
  • Oversee Gaps in Care of patients
  • Engage patients around their goals and care plan
  • Work as a team with medical assistance, Medical Liaison, Social Worker and Behavioral Health Specialists to ensure patients receive great care
Recommended Skills:
  • Proven experience as a Nurse Practitioner/Registered Nurse in a clinic, hospital, ER, or other setting
  • Ability to engage and connect with a patient and their family
  • Expertise in primary care, acute care, and chronic care practices
  • Knowledge of health and patient care regulations
  • Excellent communication skills

Day:
Nurse Practitioners at Catalytic Health Partners can expect to spend their day seeing 5 patients in their home each day. An NP will manage a population of 100 members meeting with each of them monthly to ensure their progress to goals and life changes. Working with their team of Medical Assistance, Medical Liaisons, Social Worker, and Behavioral Health Specialist they will plan as a team the engagement and coordination necessary to change the overall projection of the patient. Along with their team they will work to ensure members have proper transitional care out of facilities and patient's specialist and PCP are involved members situation. Education All nurse practitioners are licensed to practice as a Registered Nurse with a specific license to practice as an Advanced Practice Nurse in the state of Arizona under the provisions of the Arizona State Board of Nurse Examiners and accredited by an approved certifying body for Advanced Practice for Nursing (American Academy of Nurse Practitioners or American Nurse Credentialing Center).

Salaries start at $130,000 per year with Catalytic Health Partners paying for you Medical Insurance, Dental Insurance and a Life Insurance policy for you (that is $8500 not coming out of your pay). Additionally Nurse Practitioners may earn bonuses for performance in May and November each year. The bonus can be up to 20% of the Nurse Practitioners pay.
