Culver City, CA Job Details Full-time Up to $130 an hour 8 hours ago Qualifications Teamwork Certified Nurse Practitioner BLS Certification ACLS Certification Driver's License Advanced practice provider Full Job Description We are seeking a compassionate and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our mobile healthcare team. The provider will be responsible for evaluating home health patients, developing and supervising care plans, and collaborating with home health agencies to ensure optimal patient outcomes. The ideal candidate will have experience in geriatric care, chronic disease management, and home-based healthcare services.
Key Responsibilities:
Perform comprehensive in-home patient assessments, including history-taking, physical examinations, and diagnostic evaluations. Develop, implement, and oversee individualized care plans in collaboration with home health agencies and caregivers. Provide ongoing supervision and monitoring of patient progress, making necessary adjustments to care plans as needed. Coordinate with physicians, nurses, and other healthcare professionals to ensure high-quality patient care. Educate patients and families on disease management, medications, and preventive care. Document patient visits, findings, and treatment plans in a timely manner. Ensure compliance with state and federal regulations regarding home health supervision and patient care.Compensation:

Education:
Master's or Doctoral degree in Nursing (NP) or a Master's degree in Physician Assistant Studies.Licensure & Certification :
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license in California.
- Board certification as an NP (AANP/ANCC) or PA (NCCPA).
- DEA license (or eligibility to obtain one).
