Key Responsibilities:

Conduct in-home primary care visits, wellness evaluations, and routine physical exams. Provide specialized wound care assessments and management. Manage chronic conditions, review diagnostic results, and prescribe appropriate treatments/medications. Document patient visits accurately and promptly using our electronic medical record (EMR) system. Coordinate care with our clinical support team and referring providers.

Requirements & Qualifications:

Active, unrestricted California license as a Nurse Practitioner (NP) or Physician Assistant (PA) . Active DEA license and furnishing/prescribing privileges in California. Willingness to travel to conduct patient home visits. Availability to visit patients at least 2-3 days per week.

High need for bilingual/multilingual providers:

Fluency in Spanish, Russian, or Filipino (Tagalog) is strongly preferred / high priority. Previous experience in Primary Care, Geriatrics, Home Health, or Wound Care preferred but not required. Valid CA driver's license, reliable personal vehicle, and active auto insurance. Strong autonomy, clinical assessment skills, and empathetic bedside manner. Benefits /
