In this role, you will manage a diverse outpatient panel, focusing on prevention, chronic disease management, and same-day care. You will collaborate with an interdisciplinary team, leverage advanced EHR and diagnostic tools, and participate in quality and population health initiatives. Responsibilities Provide comprehensive primary and preventive care to an adult and pediatric patient panel Manage chronic conditions following evidence-based guidelines Perform routine exams, minor procedures, and same-day sick visits Order, interpret, and act on diagnostic tests and imaging Document care accurately in the EHR and maintain regulatory compliance Coordinate care with specialists, hospitals, and community resources Participate in quality improvement and population health initiatives Educate patients and families on disease prevention and self-management Collaborate closely with nurses, MAs, and other care team members Engage in ongoing professional development and clinical conferences Required Skills Primary care clinical assessment Chronic disease management (diabetes, HTN, COPD) Preventive care and wellness counseling Electronic Health Record (EHR) documentation Care coordination and referral management Minor office procedures Population health management Evidence-based medicine and clinical guidelines Patient education and motivational interviewing Interdisciplinary team collaboration
