Job Description

In this role, you'll provide comprehensive assessments, diagnosis, and ongoing management for diverse patient populations, using evidence-based protocols and digital tools to improve outcomes. You'll work closely with hospitals and health systems using Incredible Health's tech-enabled platform to coordinate care, streamline communication, and support efficient staffing. As part of a mission-driven, healthcare-focused, and data-driven team, you'll collaborate with clinicians, operations, and product partners to refine workflows and implement practical technology that supports nurses and care teams at scale. You'll contribute to quality improvement initiatives, telehealth services, and patient education while practicing in a hybrid environment that blends in-person and virtual care.
