$120,000 - $140,000

Position Description You will serve as the primary care and rounding provider inside one skilled nursing facility in Milton, FL, managing a consistent panel of long term care and short stay rehab residents rather than traveling between buildings. This is a full time, in person position, Monday through Friday, 9 to 5, with no nights, no weekends and no on-call. You are employed by our provider network rather than the facility, so you do not report to the DON or the administrator, though you take part in patient care meetings.

Qualifications:

Active Florida NP or PA license and DEA registration Board certification as an FNP, AGNP or AGACNP, or NCCPA certification for a PA Experience in skilled nursing, long term care or geriatric primary care preferred Comfortable managing a full resident panel independently

Responsibilities:
