Position Summary The Family Medicine Nurse Practitioner (NP) provides comprehensive primary care services to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. The NP works collaboratively with physicians and the healthcare team to deliver high-quality, patient-centered care in a family medicine setting. Bilingual fluency in English and Spanish is required to effectively communicate with and care for our diverse patient population. Key Responsibilities Conduct complete patient assessments, including medical history, physical examination, and diagnostic testing. Diagnose, treat, and manage acute illnesses, chronic conditions, and minor injuries. Order, interpret, and evaluate laboratory tests, imaging, and other diagnostic procedures. Develop and implement evidence-based treatment plans, including prescribing medications and therapies within the scope of practice. Provide preventive care, health education, and counseling to patients and families. Collaborate with physicians, nurses, and other healthcare professionals to ensure continuity of care. Maintain accurate and timely electronic medical records (EMR). Participate in quality improvement, compliance, and patient safety initiatives. Refer patients to specialists and coordinate care as necessary. Stay current with medical knowledge, clinical guidelines, and continuing education requirements.
