Job Description

We're expanding our team that provides high-quality, patient-centered care to residents in skilled nursing facilities across DeKalb, IL. If you're a compassionate Nurse Practitioner or Physician Assistant who enjoys working with older adults in a collaborative, team-oriented setting, we'd love to hear from you. What the role looks like Full-time positions with flexible scheduling Six-hour facility shifts for full-time providers (rounds typically start between 7:00 a.m. and 10:00 a.m.) No overnight shifts or on-call duties Daily rounds focused on managing acute and chronic conditions, prescribing medications, and developing treatment plans Efficient documentation using a streamlined EHR with AI scribe and voice dictation Company-provided iPad with LTE for clinical use Access to UpToDate® for real-time clinical guidance Compensation and benefits Competitive pay with an uncapped, productivity-based model that lets you exceed industry benchmarks Up to $50,000 in long-term retention incentives Performance bonus program Sign-on bonus and relocation assistance available for eligible candidates Paid Time Off 401(k) with employer match Medical, dental, vision, life, and disability insurance for eligible providers CME allowance and licensure reimbursement Company-provided iPad and lab coat Where else we're hiring Opportunities also available nationwide in Massachusetts, Michigan, New Hampshire, North Carolina, Ohio, and Wisconsin.
