Key Responsibilities Clinical Care Conduct comprehensive histories and physical examinations Diagnose and manage acute and chronic conditions within scope of practice Develop and implement individualized, evidence-based treatment plans Prescribe medications and therapies as appropriate Provide preventive care, health maintenance, and wellness counseling Perform in-office procedures as appropriate to training and specialty Manage a patient panel with responsibility for continuity of care Care Coordination & Patient Support Coordinate care across specialties and settings to ensure seamless patient experiences Provide clinical guidance and follow-up for patient concerns and care needs Support patients and families in navigating health conditions and treatment plans Collaborate with interdisciplinary team members to optimize outcomes Documentation & Quality Complete accurate, timely documentation, coding, and billing Participate in quality improvement and performance initiatives Contribute to development and refinement of workflows and care pathways Professional Development Engage in ongoing learning through conferences, CME, and educational opportunities Maintain licensure, certification, and prescriptive authority Stay current with evolving clinical guidelines and best practices Team & Culture You will join a collaborative care team that includes physicians, advanced practice clinicians, nurses, and support staff.
