Job Description
Overview The University of Kansas St Francis is seeking an experienced, Full-Time BC Nurse Practitioner or Physician Assistant to join our growing team in Topeka, KS. Background in Primary care or Internal Medicine preferred.
Position Summary:
The Internal Medicine Advanced Practice Provider (APP) supports the Internal Medicine physician team by increasing patient access, improving continuity of care, and optimizing physician schedule capacity. The APP manages appropriate established-patient follow-up visits, routine chronic disease management, transitional care visits, and same-day or acute needs within the APP's scope of practice. The position works collaboratively with Internal Medicine physicians and the clinical care team to ensure patients receive timely follow-up while allowing physicians to maintain schedule capacity for new patients, medically complex patients, and visits requiring physician-level evaluation.About your role:

7-4pm Monday-Friday- Join a team of 3 physicians and 1 APP
Expected Daily Patient Volume:
18-22- Outpatient Only
- No clinic call schedule
- Manage established-patient follow-up visits for stable and appropriately selected patients.
- Provide ongoing management of common chronic conditions, including hypertension, diabetes, hyperlipidemia, and other conditions appropriate to the APP's training, privileges, and scope of practice.
- Conduct post-hospital and post-emergency department follow-up visits and support transitions of care.
- Provide same-day and acute visits when clinically appropriate to improve access and reduce unnecessary physician schedule utilization.
- Review diagnostic testing and laboratory results and provide appropriate patient follow-up.
- Manage medication monitoring, refills, and treatment adjustments within scope of practice.
- Identify patients requiring physician evaluation and escalate complex or high-risk clinical concerns appropriately.
- Collaborate with physicians to develop standardized criteria for visits that can be appropriately scheduled with the APP.
- Coordinate referrals, diagnostic testing, and follow-up care with the broader care team.
- Provide patient education regarding diagnoses, medications, preventive care, and treatment plans.
- Complete accurate and timely documentation, coding, and charge capture in the electronic medical record.
- Participate in quality, access, patient experience, and population-health initiatives.
