Job Description
Overview We are seeking a dynamic and compassionate Nurse Practitioner specializing in Skilled Nursing Facilities (SNF), Long-Term Care, and Subacute Care settings. In this vital role, you will deliver comprehensive patient-centered care to diverse populations, including elderly residents and individuals requiring ongoing medical management. Your expertise will help improve quality of life, facilitate recovery, and promote optimal health outcomes within a collaborative healthcare environment. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine appropriate care plans. Prescribe medications, manage chronic conditions, and perform procedures such as suturing, IV infusions, catheter care, and wound management in compliance with state regulations and collaborative practice agreements. Coordinate with interdisciplinary teams—including nursing staff, therapists, and primary care providers—to develop and implement individualized treatment strategies. Oversee discharge planning, medication reconciliation, and documentation within Electronic Health Record (EHR) systems like Epic or eClinicalWorks to ensure accurate medical records. Perform specialized procedures such as gastrostomy tube feeding management, spinal taps, or dermal fillers as needed for patient comfort and recovery. Engage in telehealth consultations when appropriate to extend care beyond the facility setting and improve access for patients with mobility or transportation challenges. Ensure compliance with HIPAA regulations, infection control protocols, and safety standards while delivering high-quality patient care. Skills Proven experience working with elderly populations in nursing homes or long-term care facilities; familiarity with subacute care environments is highly desirable. Strong knowledge of EMR/EHR systems such as Epic, Cerner, or Athenahealth for efficient documentation and patient tracking. Skilled in performing clinical procedures including phlebotomy, venipuncture, sterile processing techniques, and basic life support (BLS). Expertise in managing complex cases involving acute pain management, infectious disease care, hospice & palliative medicine, and geriatrics. Ability to collaborate effectively with healthcare professionals across disciplines to optimize patient outcomes through case management and discharge planning. Familiarity with coding systems such as CPT (Current Procedural Terminology), ICD-10/ICD-9 for accurate documentation of diagnoses and procedures. Excellent client communication skills to educate patients and families about treatment plans while demonstrating empathy and professionalism. Knowledge of Workers' Compensation law and utilization review processes relevant to post-acute care settings.
