Full Job Description Job Overview:
Vitality Visiting Physicians PLLC is dedicated to providing high quality, compassionate in-home care to our patients throughout the Metro-Detroit area. We are currently looking to hire a reliable and friendly Nurse Practitioner to join our professional team. Highly-flexible scheduling options available.Responsibilities:
- Perform assessments, order labs and diagnostic tests, diagnose and treat patient conditions, manage patient medications, refer to specialists, order DME as indicated.
- Practice preventative care (wellness exams, health screenings, etc.) according to established guidelines.
- Collaborate with the healthcare team to develop and implement care plans including
PT/OT/ST.
- Offer emotional support and education to patients and their families.
- Follow up with patients to maintain quality care outcomes and respond promptly to patient concerns and/or issues.
Qualifications:
- Must hold a current Michigan NP license in good standing.
- Knowledge of medical record documentation and maintaining confidentiality.
- Strong communication skills and ability to work well within a fast-paced team environment.
Pay:
$80.00- $110.
