We're expanding a team that delivers high-quality, patient-centered care to residents in skilled nursing facilities across Ossipee, New Hampshire. If you're a compassionate Nurse Practitioner or Physician Assistant who enjoys working with older adults in a collaborative, team-based setting, we'd love to hear from you. What you'll do Provide daily rounds in skilled nursing facilities, managing acute and chronic conditions, prescribing and adjusting medications, and developing treatment plans. Work efficiently with a streamlined EHR (AI Scribe enabled) and voice dictation to document care. Use company-provided tools - including an iPad with LTE - and clinical resources like UpToDate® for point-of-care guidance. Coordinate with interdisciplinary teams to support residents' overall health and quality of life. Schedule and work environment Full-time positions with flexible scheduling. Typical full-time shifts are six hours; rounds usually start between 7:00 a.m. and 10:00 a.m. No overnight or on-call responsibilities. Board certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent). Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred.
