IHA Per Diem Nurse Practitioner / Physician Assistant Reports to:

Commitment:

Minimum of 15 completed visits per month Position Summary Perform in-person, no-cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act (ACA) members. Assessments are conducted within the member's personal living space, ensuring high-quality care delivery according to market health plan expectations.

Hours & Compensation Flexible Scheduling:

Days and evenings available.

Appointment Time:

60-75 minutes per assessment.

Compensation:

Competitive market rate per completed assessment.

Mileage:

Reimbursed at the IRS Standard Mileage Rate after the first 30 miles driven per day.

Essential Functions Availability:

Provide a minimum of 15 appointment slots per month.

Clinical Excellence:

Perform high-quality assessments that meet member satisfaction and health plan quality standards.

Documentation:

Complete and submit chart notes within 24 hours of the visit; address charting queries within 48 hours.

Communication:

Maintain proactive communication with clinical leadership and attend periodic training sessions.

Experience & Education Required Experience:

Primary Care (PCP) or In-Home Health experience is required.

Home Health History:

Must have home health experience within the last 5 years.

Preferred:

1 year of specific in-home health assessment experience is highly preferred.

Education:

Master's or Doctorate from an accredited NP program (FNP, AGNP, ANP) or a Master's from an accredited PA program.

Competencies Risk Adjustment:

Understanding of CMS and ACA risk adjustment models.

EMR Proficiency:

Strong computer skills and proficiency with electronic medical records.

Communication:

Excellent verbal and written skills with a positive, collaborative attitude.

Compliance:

Maintain patient confidentiality in accordance with HIPAA regulations.

Problem Solving:

Ability to adapt to changing conditions and proactively address member, provider, or health plan issues.

Certification & Licensure Licensure:

Current RN and NP or PA licensure in NY State.

Board Certification:

ANCC, AANP, or NCCPA certification.

Credentials:

Must have a current State Medicaid ID and Medicare PTAN Number to prevent credentialing delays (Preferably credentialed with Excellus).

Safety:

Current BLS certification.

Driving:

Valid, unencumbered state driver's license and proof of sufficient auto insurance. Medical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.
