We are seeking a Mid-Level Provider (Physician Assistant or Nurse Practitioner) for an in-person outpatient role in Olean, NY. Assess, diagnose, and manage acute and chronic conditions while building patient relationships. Perform exams, obtain histories, administer medications, and document care using Medent EMR. Educate patients on health, medications, and wellness, providing health coaching for better outcomes. Qualifications include an active NY State PA or NP license, strong clinical skills, and familiarity with outpatient care; telehealth experience is a plus.
