$76-$78 per hour Lorain, OH Contract

• Position Overview
• The Nurse Practitioner (NP) will provide high-quality, evidence-based primary and family care within a collaborative clinical care team.

This role focuses on delivering comprehensive patient care, improving health outcomes, and supporting patients in managing acute and chronic conditions. The NP will work closely with a multidisciplinary team while maintaining autonomy in clinical decision-making.

• Key Responsibilities:

• + Provide primary and family care, including assessment, diagnosis, treatment, and management of health conditions + Deliver patient-centered care, including health counseling and disposition planning + Conduct routine exams, diagnostic testing, and treatment for acute and chronic conditions + Promote preventive care and healthy lifestyle choices through screenings and wellness education + Manage chronic disease conditions and coordinate care plans with other healthcare professionals + Encourage patient engagement in their own health and well-being + Perform administrative duties including: + Clinic opening/closing procedures + Inventory management + Insurance verification and payment collection + Patient follow-ups and appointment management + Maintain accurate documentation and ensure a clean, compliant clinical environment •

Required Qualifications:

• + Master's Degree from an accredited Family Nurse Practitioner (FNP) program + Current National Board Certification + Active, unrestricted State NP license + Minimum 1+ year of recent Primary Care, Family Medicine, or Urgent Care experience as an NP + Strong knowledge of chronic disease management and quality measures + Excellent communication and patient counseling skills + Ability to work independently and within a team-based care model + Proficiency with EMR systems and healthcare technology •

Preferred Qualifications:

• + Experience in value-based care models + Willingness to obtain multi-state licensure + Strong organizational, multitasking, and problem-solving skills
