Job Description
Seeking an Advanced Practice Provider to deliver outpatient primary care at a Family Care Clinic in Prineville, OR. Under supervision, provide comprehensive medical care across all ages, including evaluation, diagnosis, treatment, and documentation. Coordinate with behavioral health, pharmacy, nursing, and other services within a Medical Home model to optimize patient outcomes. Create and monitor treatment plans, referring patients to specialists as needed. Work a minimum of 40 hours weekly, with scheduled patient contact and participation in a shared call rotation, including evenings, weekends, and holidays. Collaborate with a multidisciplinary team, uphold clinical standards, and ensure regulatory compliance. Maintain current licensure, certifications, and participate in ongoing education. Ideal candidates have outpatient family medicine experience, EHR proficiency, and relevant licenses. Preferred candidates have 2+ years experience, familiarity with Medical Home, and Epic EHR.
