Job Description

Job Summary We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will provide comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of chronic conditions. This role requires a strong commitment to patient-centered care and the ability to work collaboratively within a multidisciplinary team. The ideal candidate will be proficient electronic health record systems. We are a Tier 5 PCPCH through OHA and are constantly striving to better patient experience, patient screenings and be the primary care clinic of choice in the area. We believe in a work-life balance! We are also the 1st primary care clinic in Oregon to receive the Health Equity Designation! Duties Conduct thorough patient assessments, including physical examinations and health histories for all ages Develop and implement individualized care plans based on patient needs and clinical guidelines. Provide primary care services, including diagnosis and treatment of acute and chronic illnesses. Perform preventive health screenings and immunizations. Educate patients and families about health management, lifestyle modifications, and disease prevention. Collaborate with other healthcare professionals to coordinate patient care across various settings, including clinics and hospitals. Maintain accurate and up-to-date patient records in compliance with HIPAA regulations. Participate in clinical research initiatives as needed to enhance patient care outcomes. Utilize telehealth technologies to provide remote consultations when appropriate. Requirements Master's degree in Nursing from an accredited program. Current certification as a Family Nurse Practitioner (FNP). Valid state nursing license with prescriptive authority. Experience in a clinical setting, preferably in family practice or internal medicine. Familiarity with electronic health record systems, particularly Athena. Strong communication skills and the ability to build rapport with patients of diverse backgrounds. Commitment to maintaining confidentiality and adhering to HIPAA regulations. Ability to work effectively both independently and as part of a healthcare team.
