Job Description

Job Summary We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. In this vital role, you will travel to patients houses, group homes and Assisted living facilities, to deliver comprehensive primary care services. The services will be focusing on promoting health, preventing illness, and managing acute and chronic conditions across diverse patient populations. Your expertise will empower adults and geriatrics— ensuring they receive personalized, high-quality care in a supportive environment. This position offers an exciting opportunity to utilize your broad clinical skills, including medical documentation and patient assessment, to make a meaningful difference in our community's health and well-being. Mileage will be reimbursed. Duties Conduct thorough physical examinations and health assessments for adults and geriatrics, utilizing your physiology knowledge and clinical evaluation skills, in Assisted living facilities and homes. Diagnose and treat conditions. Develop and implement individualized treatment plans. Utilize EMR (Electronic Medical Records) systems, eClinicalWorks, to document patient encounters accurately while maintaining HIPAA compliance. Provide patient education on wellness strategies, chronic disease management and nutrition. Collaborate with specialists in gastroenterology, infectious disease care, hospice & palliative medicine, geriatrics, or occupational health to optimize patient outcomes. Participate in clinical research projects or quality improvement initiatives to advance evidence-based practices within the practice. Requirements Valid Family Nurse Practitioner license with current certification from an accredited program. New grads welcome.

We are willing to train. Strong knowledge of medical terminology. Experience with telehealth platforms. Excellent communication skills with the ability to educate patients effectively and geriatrics (memory/dementia care). Knowledge of workers' compensation law and occupational health protocols is beneficial. Demonstrated ability to work independently while collaborating within a multidisciplinary team committed to delivering exceptional patient-centered care. Pay:
$116,017.04 - $139,719.44 per year Willingness to travel: 75% (Preferred)
