CORNERSTONE
CARES, our new in-home behavioral health program for children, adolescents, and families. CARES is designed to help bridge gaps in continuity of care for children who need additional behavioral health support while waiting for a higher level of care or transitioning back to outpatient and community-based treatment. The Psychiatric APRN will provide psychiatric evaluation and medication management for identified CARES clients when clinically appropriate and requested by the family, working collaboratively with the child's CARES In-Home Clinician. In-Home Component This is a community-based position with an in-home component. The initial APRN session with the child/adolescent and family will take place in the family's home. Follow-up services may be provided in-home or through other appropriate service modalities based on clinical need, program requirements, and applicable payer requirements. We are seeking an APRN comfortable traveling within one or more of our initial CARES service regions: Greater Waterbury; Naugatuck Valley; and Greater Plainville / Southington / Berlin area. Responsibilities Conduct psychiatric evaluations for children and adolescents Provide psychiatric medication management and follow-up Complete the initial psychiatric visit with the child/adolescent and family in the home Provide additional in-home visits when clinically appropriate Prescribe and monitor psychiatric medications within scope of practice Collaborate closely with CARES In-Home Clinicians Coordinate with parents/guardians and authorized treatment providers Monitor treatment response, side effects, adherence, risk, and safety concerns Complete timely and accurate electronic documentation Participate in care coordination with pediatricians, outpatient providers, hospitals, IOPs, schools, and other community providers when appropriate Follow Cornerstone clinical, documentation, confidentiality, and community-based safety requirements Qualifications Current, unrestricted Connecticut APRN license Appropriate psychiatric certification and prescriptive authority within scope of practice Experience working with children, adolescents, and families strongly preferred Comfortable providing services in family homes and community settings Strong psychiatric assessment and medication-management skills Ability to work independently while collaborating with a multidisciplinary treatment team Reliable transportation and ability to travel within an assigned service area Strong documentation, communication, organization, and clinical judgment Bilingual English/Spanish skills preferred but not required Why Cornerstone $80-$100/hour with mileage reimbursement Flexible scheduling Referrals provided Administrative and billing support Insurance credentialing/paneling support Electronic health record and technology support Collaborative relationship with CARES In-Home Clinicians Opportunity to help launch a new community-based behavioral health program Meaningful work addressing behavioral health access barriers for children and families
