As a Nurse Practitioner with Carease Health you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. We're seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team. About the
Role:
As a Nurse Practitioner with Carease Health you will: Deliver comprehensive preventative care to residents in skilled nursing facilities Conduct on-site visits and provide patient-centered care, supported by remote monitoring and chronic care management Build strong relationships with facility staff and patients, maintaining a patient-centered approach Collaborate with our clinical directors, regional managers, and supervisors to ensure high-quality care Participate in quality improvement initiatives to minimize hospitalizations and elevate patient care standards
Requirements:
Active NP or PA license (in the state of practice) 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills
