Texas Post Acute Care is expanding and seeking a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing clinical team. This position offers the independence and autonomy of private practice with the support of a collaborative healthcare environment. The provider will care for patients at partner post-acute and skilled nursing facilities throughout Northeast San Antonio to Austin, TX .

Responsibilities:

Perform comprehensive patient assessments, histories, and physical examinations Diagnose and manage acute and chronic medical conditions Develop and implement evidence-based treatment plans Prescribe medications and monitor patient response and progress Order and review appropriate laboratory testing and diagnostic studies Educate patients and families regarding diagnoses, medications, and treatment plans Collaborate with physicians, nursing staff, therapists, and other members of the interdisciplinary care team Assist with transitions of care and discharge planning Maintain timely and accurate EHR documentation Travel daily to assigned partner facilities within the designated service area

Qualifications:

Active Texas Nurse Practitioner (NP) or Physician Assistant (PA) license Master's degree from an accredited NP or PA program Current board certification DEA registration or ability to obtain Strong clinical judgment, communication, and organizational skills Ability to work independently while collaborating with a healthcare team Reliable transportation and ability to travel locally between assigned facilities Post-acute care, skilled nursing, internal medicine, family medicine, hospitalist, or geriatric experience is preferred but not required. $125,000.00 per year
