Nurse Practitioner (Primary Care & Post-Hospital Follow-Up)
Status:
Part-Time (1-3 days/week) Job Overview Are you a Nurse Practitioner looking for a flexible, part-time opportunity focused on delivering high-quality primary care and post-hospital follow-up services? This position allows you to provide personalized, patient-centered care throughout the Hampton Roads region while maintaining a schedule that fits your availability. The organization is open to providers who can work 2-3 days per week. Role Overview In this role, you will provide comprehensive primary care and transitional care services to patients in their homes and senior living communities. You'll perform post-discharge follow-up visits, manage chronic and acute medical conditions, and work collaboratively with care teams to improve patient outcomes while reducing hospital readmissions. Key Responsibilities Deliver primary care and post-hospital follow-up services in patients' homes and senior living communities Perform comprehensive health assessments, follow-up evaluations, and ongoing care management Conduct post-discharge visits to support safe care transitions and reduce readmission risk Diagnose medical conditions and develop individualized treatment plans Prescribe medications and order appropriate diagnostic testing Coordinate referrals and collaborate with interdisciplinary care teams Educate patients and caregivers on disease management, medications, and preventive care Monitor patient progress and modify treatment plans as clinically appropriate Complete all EMR documentation within 24 hours of each patient encounter Consult with the Medical Director or collaborating providers as needed Schedule Part-time, 2-3 days per week Flexible scheduling Opportunity for providers interested in working 1-2 days per week Typical work hours: 8:00 AM - 5:00 PM No on-call responsibilities Orientation and onboarding provided Credentialing timeline approximately 30 days Service Areas Virginia Beach, VA Chesapeake, VA Hampton, VA Newport News, VA Required Qualifications Active Virginia Nurse Practitioner license Current DEA registration Active CAQH profile and CAQH number Minimum of one year of Nurse Practitioner experience in primary care, family medicine, senior care, assisted living, long-term care, or transitional care Experience utilizing electronic medical record (EMR) systems Strong clinical assessment and independent decision-making skills Preferred Qualifications Experience providing post-acute or transitional care services Background in urgent care, emergency medicine, hospital medicine, or acute care Experience caring for geriatric and medically complex patient populations Key Skills Required Primary care assessment and diagnosis Post-hospital transitional care management Chronic disease management Care coordination EMR documentation Patient and caregiver education Clinical decision-making Why This Location?
