- Responsibilities Conduct thorough psychiatric evaluations and mental health assessments for patients in post-acute and long-term care settings.
Prescribe and manage medications in accordance with state regulations, including psychopharmacology treatments such as antidepressants, antipsychotics, and mood stabilizers. Collaborate with healthcare professionals across disciplines—such as physicians, advanced practice clinicians, social workers, and therapists—to coordinate holistic patient care plans. Document patient encounters accurately using electronic health record (EHR) systems ensuring compliance with HIPAA regulations. Participate in discharge planning, case management, and follow-up care to optimize patient outcomes during transitions of care. Educate patients and families about mental health conditions, medication management, and wellness strategies to promote recovery and stability. Requirements Current license as a Nurse Practitioner with certification in Psychiatric-Mental Health (PMHNP) from an accredited program. Proven experience working with vulnerable populations in settings such as nursing homes, assisted living facilities, or post-acute care environments. Strong knowledge of psychotropic medications, including their indications, contraindications, and side effects. Familiarity with EMR/EHR systems such as Athenahealth for documentation and clinical decision support. Ability to perform comprehensive patient assessments including vital signs, mental status exams, and medication reconciliation. Experience managing complex cases involving behavioral health issues, substance use disorders, or dementia-related conditions like Alzheimer's disease or memory care needs. Excellent communication skills to collaborate effectively with multidisciplinary teams and engage patients compassionately.
