Responsibilities Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and manual handling techniques. Monitor vital signs including blood pressure, pulse, temperature, and respiration; document findings accurately in electronic health record (EHR) systems. Support medication administration under supervision, ensuring proper handling and adherence to HIPAA regulations to maintain patient confidentiality. Assist with medical procedures such as catheter care, wound dressing changes, phlebotomy (blood draws), and aseptic techniques to prevent infection. Observe and report changes in patient condition promptly to nursing staff; participate in patient transfer methods ensuring safe handling practices. Maintain detailed medical documentation using EMR/EHR systems like Cerner or eClinicalWorks; ensure all records are accurate and up-to-date. Provide emotional support and effective communication tailored to patients' needs, including those with dementia or Alzheimer's disease, fostering a caring environment. Skills Proven experience working with geriatric patients in long-term care facilities or skilled nursing environments. Knowledge of medical terminology, anatomy, physiology, and basic life support (BLS) including CPR certification. Familiarity with EMR/EHR systems such as Epic or Meditech; experience managing electronic health records efficiently. Ability to perform heavy lifting safely while adhering to safe lifting practices; proficient in manual handling techniques. Strong caregiving skills with experience supporting individuals with developmental disabilities, dementia care, memory care, hospice care, or ICU experience. Excellent communication skills for caregiver-patient interactions and interdisciplinary team collaboration.
