Assistant DSD/LVN - Skilled Nursing Facility

Job Description

Overview We are seeking a dedicated and highly skilled Assistant DSD/LVN (Licensed Vocational Nurse) to join our esteemed Skilled Nursing Facility. This vital role requires a professional committed to delivering exceptional patient care while supporting clinical operations and staff development. The ideal candidate will possess extensive experience in adult and acute care settings, including nursing homes, hospitals, or ICU environments, with a strong understanding of healthcare regulations and medical documentation. As an integral part of our team, you will contribute to maintaining high standards of patient safety, comfort, and quality care through effective clinical instruction and team leadership. Duties Provide direct patient care in accordance with physician orders, ensuring safety, comfort, and dignity for residents across various acuity levels. Assist registered nurses and physicians with clinical procedures, medication administration, and vital sign monitoring. Support the training and development of clinical staff through education on medical terminology, physiology, anatomy, and health regulation policies. Maintain accurate and comprehensive medical records in compliance with HIPAA and state healthcare regulations. Participate in patient assessments, documentation, and reporting to facilitate effective care planning. Collaborate with multidisciplinary teams to implement individualized care plans that address both acute and chronic conditions. Contribute to hospital or nursing home quality improvement initiatives by adhering to best practices in patient care and safety protocols. Lead or assist in precepting new staff members or students in clinical instruction programs focused on adult learning principles. Qualifications Valid LVN license with current registration; additional certifications in critical care or ICU nursing preferred. Proven experience in nursing home settings, hospital environments, or ICU units with a focus on adult patients. Demonstrated knowledge of medical terminology, physiology, anatomy, and healthcare regulations including HIPAA and state policies. Strong proficiency in medical documentation systems such as electronic health records (EHR) using Microsoft Office tools for reporting and record-keeping. Experience in teaching hospital or home care environments is advantageous for comprehensive clinical instruction skills. Ability to lead clinical teams effectively while fostering a collaborative learning environment through training & development initiatives. Excellent communication skills combined with a thorough understanding of patient care standards across diverse healthcare settings. Prior experience with nursing leadership or team supervision is highly desirable for guiding staff performance and ensuring compliance.
