We are seeking two adaptable and skilled (CNA / HHA / Caregiver) to join our home health team in Union City. In this vital role, you will deliver essential, high-touch clinical and personal care directly to seniors within the East Bay community. Operating with a high degree of autonomy, you will manage vital tasks including patient transfers, daily living assistance, and clinical monitoring. If you are a reliable caregiver who holds active clinical certifications, values a flexible schedule, and wants to build meaningful one-on-one relationships with older adults, we have an immediate placement for your expertise!
Duties & Responsibilities Direct Patient Care:
Assist senior clients with essential activities of daily living (ADLs), including bathing, grooming, dressing, and nutritional support.
Mechanical Transfers:
Safely and proficiently operate a Hoyer lift and other assistive devices to transfer, position, and ambulate patients with limited mobility.
Clinical Safety & Monitoring:
Deliver basic supportive care in strict alignment with home health agency policies, keeping alert for any changes in a client's health or living situation.
Emergency Response:
Maintain immediate readiness to perform life-saving CPR or First Aid procedures to guarantee client safety until advanced medical services arrive.
Operational Flexibility:
Commit to a baseline of 20 or more hours per week, maintaining the personal flexibility to pick up additional hours or extended shifts when department needs arise.
Qualifications Credentials:
Active, valid state certifications as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA).
Life Support:
Current certifications in CPR and First Aid.
Technical Skill:
Proven professional experience working with a senior population and verified, hands-on proficiency operating a Hoyer lift .
Mobility:
A valid driver's license is required along with a reliable track record of safe driving.
Desired Qualifications Transportation:
Access to your own personal vehicle for efficient travel between community home sites is highly preferred.
Adaptability:
Prior experience navigating independent home health environments or community public health programs.
Language Requirement:
Fluent bilingual communication skills in English AND at least one of the following: Cantonese, Mandarin, or Hindi are preferred.
Location and Work Type Work Type:
Part-Time Direct Placement (Permanent Positions).
Commitment:
Ability to commit to a minimum of 20+ hours per week with schedule flexibility.
Shift Availability:
Must be fully available Monday through Sunday, 7:00 AM - 7:00 PM. Actual shift times vary according to department and patient needs, including required evenings, weekends, and holidays.
Work Setting:
100% Mobile field-based environment, delivering care directly inside client residences.
Location:
Union City, CA (East Bay Community region).
