Duties Deliver direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Support patients with meal preparation, light housekeeping including cleaning and laundry, and ensuring a safe environment aligned with home healthcare regulations. Document medical records meticulously in electronic medical record (EMR) systems; communicate effectively with healthcare teams regarding patient status and care plans. Qualifications Valid certification as a CNA, HHA, or PCA with current licensing or registration as required by state law. Proven experience working with seniors, individuals with developmental disabilities, or those requiring memory care or dementia support. Knowledge of medical terminology, medication administration, vital signs measurement, catheter care, and basic life support (BLS) including CPR. Strong communication skills for effective client interaction and team collaboration; ability to maintain patient confidentiality at all times. Prior experience in nursing homes or assisted living facilities is highly desirable; familiarity with hospice care and end-of-life procedures is a plus.
