Responsibilities Provide comprehensive patient care by performing assessments, vital sign monitoring, and physical examinations in diverse clinical settings. Administer medications, injections, IV infusions, and manage aseptic techniques with precision and adherence to safety protocols. Assist with complex procedures such as suturing, catheterization, tube feeding, ventilator management, and wound care including ostomy care. Support diagnostic evaluations by collecting specimens for laboratory testing, processing samples accurately, and documenting results within electronic health record (EHR) systems like PointClick Care, Epic or Cerner. Collaborate with multidisciplinary teams on discharge planning, case management, and patient education to ensure continuity of care across settings. Manage acute pain and infectious disease cases while practicing infection control standards aligned with CDC guidelines. Utilize critical thinking skills during triage assessments in urgent care or emergency room environments to prioritize patient needs effectively. Qualifications Valid licensure as an RN, LPN/LVN, or CNA in the state of CT. Physical Requirements Candidates must be able to perform the physical demands of the role, including standing and walking for extended periods, bending, lifting, and repositioning patients (up to 50 lbs with or without assistance), and responding quickly in fast-paced clinical environments, with or without reasonable accommodation. Proven experience in one or more specialized areas such as LTC, critical care (ICU/Trauma), emergency medicine, cardiology (cardiac catheterization), nephrology (dialysis), geriatrics (memory care & long-term care), pediatrics (neonatal & pediatric ventilator), or behavioral health. Familiarity with EMR/EHR systems such as Point Click Care, Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation. Knowledge of medical terminology, anatomy and physiology relevant to diverse patient populations including those with disabilities or requiring hospice & palliative care. Ability to perform invasive procedures like venipuncture, IV insertion, phlebotomy and sterile processing techniques confidently. (Specific Role Dependent) Strong communication skills for effective patient assessment and health coaching across age groups from toddlers to seniors.
