CNA License (valid for 5 years) OR 75-hour HHA Certificate with Course Curriculum Transcript Certifications (Current within 2 years unless otherwise stated): CPR/BLS Card Domestic Violence Training OSHA or Infection Control Training Alzheimer's Disease and Dementia Related Disorders Assist with Self-Medication Certification

HIV/AIDS

Health & Screening:

Physical exam within last 6 months (must state "Free of Communicable Disease") TB Test (valid for 2 years) or Chest X-Ray (valid for 5 years)
