In this vital role, you will provide personalized, high-quality support to individuals across various care settings, including home care, assisted living, nursing homes, and hospitals. This position offers an exciting opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those experiencing memory care challenges such as Alzheimer's disease and dementia. This job would require qualified candidates to work in different locations throughout the following counties: Paulding, Douglas, Cherokee, Cobb, Fulton, Dekalb, Henry, Rockdale, Gwinnett and Clayton. Duties Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts. Monitor vital signs including blood pressure, pulse, temperature, and respiration; accurately document findings in electronic health record (EHR) systems. Support medication administration under supervision, ensuring proper handling and adherence to HIPAA regulations for patient privacy. Assist with specialized care needs such as catheter care, wound dressing changes, aseptic techniques, and infection control protocols. Collaborate with interdisciplinary teams in acute care environments like ICU or Level I Trauma Centers; participate in patient rounds and communicate patient status effectively. Provide compassionate companionship and emotional support to patients with behavioral health conditions or cognitive impairments such as Alzheimer's or dementia. Maintain a safe environment by following infection control standards, proper use of PPE (personal protective equipment), and adhering to medical documentation procedures. Drivers License Valid Vehicle Registration Valid Car Insurance Experience working in diverse healthcare settings including hospitals (ICU, emergency medicine), nursing homes, assisted living facilities, or home health environments. Knowledge of medical terminology, anatomy, physiology, and basic life support techniques. Familiarity with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or similar platforms for accurate documentation. Prior experience with specialized care areas including geriatrics, pediatrics, behavioral health, hospice care, or post-acute/sub-acute settings is highly desirable. Ability to work effectively with individuals with disabilities or developmental challenges; experience in memory care or dementia-specific environments is a plus.
