Job Description
We are seeking a compassionate Hospice Care Nurse (CN) to join our dedicated team. As a Hospice CN, you will provide essential end-of-life care and support to patients and their families during a challenging time.
Duties:
- Deliver high-quality nursing care to hospice patients in their homes or hospice facilities
- Administer medications and treatments as prescribed by the physician
- Monitor patient conditions and report any changes to the healthcare team
- Provide emotional support and comfort to patients and their families
- Collaborate with interdisciplinary teams to ensure holistic care for patients
- Maintain accurate patient records and documentation
Experience:
- Valid CN license in the state of practice
- Previous experience in hospice or palliative care is preferred
- Strong communication and interpersonal skills
- Ability to provide compassionate end-of-life care
