We are seeking a compassionate Hospice Care Nurse (CN) to join our dedicated team. As a Hospice CN, you will provide essential end-of-life care and support to patients and their families during a challenging time.

• Duties:

• Deliver high-quality nursing care to hospice patients in their homes or hospice facilities
• Administer medications and treatments as prescribed by the physician
• Monitor patient conditions and report any changes to the healthcare team
• Provide emotional support and comfort to patients and their families
• Collaborate with interdisciplinary teams to ensure holistic care for patients
• Maintain accurate patient records and documentation

• Experience:

• Valid CN license in the state of practice
• Previous experience in hospice or palliative care is preferred
• Strong communication and interpersonal skills
• Ability to provide compassionate end-of-life care
