Job Description

PRIMARY RESPONSIBILITIES
Direct Caregiver for individuals in CLS setting. § Assist with all ADLs § Provide supervision with taking their medication. § Assist with Routine hygiene care throughout the day. § Supervise in addressing their ISP/Plan of Care goals and document progress § Support all opportunities for the growth, learning and development of the individual § Complete daily progress notes highlighting person center events § House-keeping-- vacuuming the carpet, cleaning the toilet / bathrooms, sweeping / mopping the floors, laundry etc. Preparing a well-balanced meals and snacks § Empty trash from home at end of shift or as necessary § Complete understanding of home policies and procedures including home rules, emergency procedures, etc. § Attend all staff meeting (required); participate in scheduled training sessions including on the job training and maintain all required certifications § Promote harmonious relationship with other staff members § Maintain individuals records on site in accordance with policy and procedures § Transportation to appointments, shopping, and other outings as needed § Other duties as assigned
MININUM QUALIFICATIONS
§ Must be 18 years of age or older § CPR and First Aid certified § Negative TB test § Clean Criminal Record Check § Valid driver's license / Satisfactory driving record (per DBHDD policy)
COMPETENCY REQUIRED
§ Excellent administrative skills § Must be able to read and write English § Communicate effectively with service monitors, staff, families and state administrators § Excellent interpersonal skills

$14-$15
