Job Summary Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) / Home Health Aide (HHA), where your dedication will directly enhance the quality of life for our clients. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and document patient vital signs such as blood pressure, pulse, temperature, and respiration rates using Electronic Health Record (EHR) systems Support medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards Observe and report changes in patient condition or behavior, including patient observation for safety concerns or signs of deterioration Help with medical procedures such as catheter care, and assisting with end-of-life or hospice care needs Transfer patients safely using patient transfer methods like Hoyer lifts or other assistive devices; ensure proper body mechanics during heavy lifting or repositioning Perform light housekeeping duties including cleaning, laundry, meal preparation, and maintaining a safe environment for clients Requirements Valid certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA), with current state licensure if applicable Proven experience working with geriatric patients, individuals with developmental disabilities, dementia or Alzheimer's care experience preferred Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Knowledge of home health care assistance techniques including patient transfer methods and medication management Ability to communicate effectively with patients, families, and healthcare team members; compassionate caregiving mindset Basic life support (BLS) certification and first aid training required; CPR certification preferred Physical ability to perform tasks such as heavy lifting, assisting with mobility, and long periods of standing or walking Understanding of home health regulations, medical terminology, HIPAA compliance, and patient confidentiality standards This role is ideal for dedicated caregivers passionate about providing exceptional patient-centered care in a supportive environment.
