The ideal candidate will possess a strong commitment to patient care, demonstrate excellent communication skills, and have experience working with diverse populations, including geriatric and developmentally disabled individuals. FRIDAY-SUNDAY 6AM-6

30PM Responsibilities Provide direct patient care including assistance with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts. Monitor and record vital signs accurately while maintaining detailed medical documentation within electronic health record (EHR) systems. Assist with patient transfer methods ensuring safe manual handling practices in compliance with safety protocols. Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, or behavioral health requirements. Maintain patient confidentiality in accordance with HIPAA regulations and uphold the highest standards of infection control using aseptic techniques. Collaborate effectively with nursing staff and other healthcare professionals to ensure comprehensive patient service and continuity of care. Skills Proven experience working in medical settings such as long-term care facilities, hospitals, assisted living communities, or hospice environments. Familiarity with EMR/EHR systems Knowledge of medical terminology, physiology, and patient transfer methods to ensure safe handling of patients across various care environments Experience working with individuals with disabilities or developmental challenges; background in geriatrics or senior care is highly valued. Ability to perform heavy lifting safely while adhering to safe lifting practices; proficiency in vital signs monitoring and basic patient assessments. Strong caregiver communication skills that foster trust and comfort among patients and their families.
