This flexible, as-needed role offers you the opportunity to provide high-quality, personalized care to individuals receiving hospice services in their homes or care facilities. Your role will focus on delivering comfort, dignity, and support, ensuring patients' physical and emotional needs are met with professionalism and kindness. Duties Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility support using appropriate patient transfer methods such as Hoyer lifts. Monitor and record vital signs accurately, maintaining detailed medical documentation in compliance with HIPAA and electronic health record (EHR) systems. Provide compassionate care tailored to patients with complex conditions such as dementia, Alzheimer's disease, or other memory care needs. Support medication administration under supervision, ensuring proper handling and aseptic techniques. Collaborate with interdisciplinary teams to coordinate patient care plans, emphasizing safe lifting practices and infection control procedures. Assist with specialized hospice care tasks, including catheter care and symptom management, while maintaining patient confidentiality at all times. Utilize EMR/EHR systems efficiently to document patient interactions and ensure seamless communication across healthcare providers. Qualifications Valid CNA certification with current state licensure. Prior experience working in hospice care or with terminally ill patients is highly preferred. Demonstrated ability to work with individuals with disabilities, including developmental disabilities and behavioral health needs. Experience in various healthcare settings such as long-term care facilities, hospitals (including ICU and trauma centers), assisted living, or home care environments. Knowledge of medical terminology, vital signs measurement, and basic life support (BLS) certification. Familiarity with EMR/EHR systems is a plus. Strong communication skills to effectively caregiver communicate with patients, families, and healthcare teams. Ability to perform heavy lifting safely and adhere to safe lifting practices during patient transfers using equipment like Hoyer lifts. Commitment to maintaining patient confidentiality according to HIPAA regulations and practicing infection control standards.
