Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and dignity. Support clients with medication reminders, assisting with vital signs monitoring, and maintaining care plans in accordance with healthcare protocols. Provide companionship and emotional support to promote mental health and social engagement. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment in compliance with health standards. Utilize specialized equipment such as Hoyer lifts to safely assist with mobility and prevent injury. Document patient observations, changes in condition, and care activities accurately while adhering to HIPAA privacy regulations. Skills Proven experience working with individuals with disabilities, developmental delays, or chronic health conditions. Knowledge of first aid, CPR certification, and emergency response procedures to ensure safety at all times. Familiarity with assisted living environments, home health services, hospice care, or group homes enhances your ability to deliver comprehensive support. Strong patient observation skills to monitor health status changes and report effectively to healthcare professionals. Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts when required. Excellent communication skills for collaborating with clients' families, healthcare teams, and case workers. Experience in meal preparation, medication management, and implementing individualized care plans aligned with the plan of care or long-term care standards.
