Full Job Description Abigail Hospice, LLC Location:
Lansing, Michigan Abigail Hospice is a growing hospice company in the Livingston County area. We are looking to add a dependable, compassionate aide to our team.Responsibilities Include:
Perform personal care (bathing, dressing, toileting, feeding, etc) according to Hospice Aide Plan of Care; Document care provided according to agency policy; Report any deviations form the Plan of Care to the Case Manager (or designee if not available); Use durable medical equipment safely; Take vital signs when on Hospice Aide Plan of Care; Foster cooperative effort among personnel by understand the functions of other persons involved in patient care and by active participation in team and staff conferences; Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Report and changes/areas of concern to Case Manager; Assist with ambulation, transfers, range of motion if documented in the Hospice Aide Plan of Care; Provide respite for patient's family/caregiver when on-site, as appropriate; Participate in QAPI activities; Participate in IDG meeting, as appropriate.Job Type:
