Job Description

Are you an experienced CNA who enjoys helping others, solving problems, and being someone your team can count on? We're looking for a Lead Care Professional to provide hands-on support to our clients and Care Professionals throughout Northeast Metro Minnesota. This position offers a set number of guaranteed hours , discussed and agreed upon in advance. The schedule includes weekdays and every other weekend , with the majority of your time spent in the field. When field support isn't needed, you'll work from our Andover office assisting with administrative tasks and team projects. What You'll Do As a Lead Care Professional, you'll be a go-to resource for both clients and Care Professionals.
Responsibilities include:
Covering call-offs and open shifts Training, mentoring, and supporting Care Professionals Conducting quality field assessments and pop-in visits Providing warm introductions between clients and Care Professionals Communicating field observations and concerns to the office Managing care notes, client profiles, and quality calls Providing hands-on care when needed Assisting with administrative tasks and office projects You'll also receive Lead Care Professional training from our nursing team to help you develop the skills needed to coach, train, and support other Care Professionals. What We're Looking For Current CNA certification required Caregiving experience and confidence providing hands-on care Dependable, professional, and personable Strong communication and organizational skills Comfortable working independently and supporting others Reliable transportation Able to adapt quickly and step in wherever support is needed A Fast-Paced Role With Variety No two days will look exactly the same. You may be covering a call-off, visiting a new client, supporting a Care Professional, or handling an unexpected situation in the field. We're looking for someone who thrives in a busy environment, enjoys variety, and likes being the person others can rely on. Serving Northeast Metro Minnesota We provide in-home care throughout Andover, Maplewood, Stillwater, White Bear Lake, Chisago, Forest Lake, Oakdale and surrounding communities.
