Summary We are looking for a PRN CNA to help provide bathing, shower assistance, and personal care for patients receiving services through our Home Health and Hospice Anywhere Care teams. This is a PRN position with flexible hours based on patient needs . We are especially looking for someone who is available to pick up visits during the 8:00 AM-4:30 PM timeframe . The CNA provides personal care according to the written Care Plan prepared by the Registered Nurse, under the supervision of the RN and as ordered by the physician. Essential Duties and Responsibilities Follows the written instructions on the Aide Assignment Sheet prepared by the Registered Nurse. Assists patients with activities of daily living, including personal hygiene, bathing, shampooing, shaving, mouth care, skin care, eating, dressing, and elimination. Assists patients with ambulation, transfers, and exercises. Performs household services that are essential to the patient's care, including personal laundry, making the bed, and other small household tasks. Helps maintain a safe and comfortable environment for the patient. Maintains a cheerful and positive attitude while providing personal care and supportive services. Reports changes in the patient's condition, emotional status, or needs to the Registered Nurse/Nursing Supervisor. Completes required documentation in the electronic medical record after each visit. Maintains patient confidentiality when using the EMR device. Attends required in-service programs and staff meetings. Qualifications, Knowledge, Skills and Abilities Current CNA certification. Prior experience working as a CNA. Previous Home Health Aide experience preferred. Hospital, nursing home, or public health nursing experience preferred. Satisfactory completion of the Home Health Aide Competency Examination. Reliable transportation to travel to patient homes. Ability to follow directions and work under the supervision of a Registered Nurse. Basic knowledge of Home Health Aide procedures and personal care. Ability to recognize and respond appropriately to safety concerns in a patient's home. Ability to work effectively with patients, families, and coworkers from a variety of backgrounds.
