Job Description

Job Summary:
A member of patient care team within the Department of Hospice and directly accountable to Hospice Director or Patient Clinical Manager. The position requires that the individual assist the professional nursing staff by performing specific nurse aide functions related to patient care to include bathing, shampooing, ambulation, transfers, tidy sick area and other duties as identified on care plan under the supervision of a registered nurse or therapist. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. The CNA prepares documentation of patient care in the home at the time of visit as applicable and submits documentation in a timely manner according to policy. Notifies RN of any changes in patient and assists in meeting needs. CNA participates in in-service and competency programs as applicable. Performs other job duties as assigned.
Performance Expectations:
Demonstrates ability to bathe patient in shower or tub using appropriate water temperature, preventing falls, and maintain privacy. Demonstrates ability to transfers patients safely utilizing safe body mechanics for yourself and patient. Demonstrates ability to document in the EMR according to policies and procedures.
Qualifications:
Education/Skills High School diploma/GED required. Completion of a Nurse Aide training program from an approved school required. If no prior certificate must successfully complete a 75-hour certification program as a Home Health Hospice Aide and demonstrate competency in Nurse Aide skills test during orientation. CPR certification required by completion of clinical orientation. Current Mississippi driver license and proof of automobile insurance required.
Work Experience:
Six months of nurse aide work experience preferred. Computer experience required. Certification/Licensure-DUE
