Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs, recording medical information, and reporting changes to nursing staff Support patients with medication reminders and basic medical tasks like catheter care and wound management Help with patient transfers using proper techniques and equipment such as Hoyer lifts or transfer belts Maintain patient confidentiality by adhering to HIPAA regulations and documenting care accurately in Electronic Health Record (EHR) systems Provide companionship and emotional support, especially for individuals with dementia or Alzheimer's disease Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a safe environment in compliance with home health care regulations Qualifications Previous experience working with geriatric patients or individuals with developmental disabilities is highly preferred Knowledge of medical terminology, medication administration, and basic life support (CPR/First Aid) is required Experience in assisted living, nursing homes, hospice or home healthcare agencies is advantageous Ability to perform heavy lifting and patient transfers safely using appropriate methods and equipment Strong communication skills to effectively interact with patients, families, and healthcare teams Familiarity with EMR/EHR systems for documentation purposes Certification as a CNA or PCA is required; ongoing training in dementia care, hospice support, or memory care is a plus Join us in providing compassionate care that enhances quality of life!
