Job Summary:
Provides leadership support to the clinical staff and the Family Medicine providers of Upstate University Medicine at T5. Participates in weekly meetings, quality control, patient reports, care coordination, and problem solving in the Family Medicine department. Provides prescribed medical treatment and personal care services to patients under the supervision of a licensed medical practitioner or health professional. Works within their full scope of practice and ensures that high quality and safety is consistently delivered. Directly reports to the practice's Nurse Manager.
Duties/Responsibilities:
Responsible for providing day to day leadership and support to the assigned clinical pod departments Oversee day to day team operations of the clinical and clerical staff in the assigned clinical pod Set clear team goals and Key Performance Indicators (KPIs) Develops and maintains a system for monitoring KPIs and report progress monthly to leadership Creates an inviting and motivating work environment and atmosphere Develop thorough Epic EMR proficiency for all aspects of the Family Medicine workflow, which may include check in, check out, rooming, authorizations, referrals, etc. Able to organize and delegate tasks appropriately and efficiently Able to act as a liaison between patients, providers, office staff and outside contacts Ensures smooth provider work-flow, solving problems and notifying staff and patients of provider delays Maintains inventory and notifies Nurse Manager of needed supplies as necessary Works closely with Nurse Manager and/or clerical staff to address chart/scheduling issues Collaborates with providers to develop workflows in Family Medicine that improve efficiency Assist providers with day to day functions, scheduling, and collaboration as needed Monitors patient satisfaction levels to ensure all patient complaints are reported to Nurse Manager and/or Practice manager to attain maximum patient satisfaction Assist nurse manager with nursing competencies, in-services, and education for all staff Manage the Family Medicine NYSIIS account including data exchange reports and VFC ordering Manage and address patient reports specific to Family Medicine Attend weekly Family Medicine meetings with Family Medicine leadership Initiate and maintain a regularly scheduled meeting with Family Medicine staff to obtain feedback, share reports, and provide updates and or in-services At times may fill in for an assigned provider nurse if needed to provide safe patient care Maintains current knowledge of health care issues and standards of practice Adheres to practice policies and procedures Performs all other duties as assigned.
Minimum Qualifications:
LPN license and 4 years experience working in an ambulatory care setting, preferably primary care. Experience with electronic medical records. Familiarity with medical office procedures and standards of medical care. Current BLS certification through the American Heart Association or the American Red Cross is required.
